Due to the complexity of the members typically targeted by the Synergy Targeted Population Management® program, we have developed a multi-disciplinary approach that enables collaboration across the care community to positively impact member health status, improve outcomes and facilitate change.

Unlike disease management programs that operate at an arm’s length from the provider network and community-based organizations, Synergy Connect “plugs in” to the care community, streamlining and integrating member care management.
Enhanced Engagement for the Difficult to Reach
Through Outreach Coordination, Synergy Connect serves to extend engagement and empowerment efforts by having a local presence in the communities that our clients serve. This premium service leverages high-volume physician practices, community health centers, shelters and group homes to locate and engage members who are otherwise unreachable through customary channels. Further, Synergy Outreach Coordinators develop relationships with providers, community resources and the health plan to encourage active member participation and close care gaps.

Increased Coordination for the Clinically Complex
With the goal of maximizing the clinical impact of the Synergy program, Care Coordination focuses on unifying clinical management support, ensuring quality care and organizing the collaborative efforts of health plan, provider and community resources available to each member. This is especially useful for those complex members in the Synergy program who are enrolled in multiple care management programs. Joint case-level reviews (Clinical Collaboration), customized referral processes and follow up help the member receive the care they need.
