Population Health Management
Health Integrated’s population health management programs are centered around our guiding principle to deliver clinically sound programs with a whole person orientation. Paramount is our ability to identify populations at risk for both medical and psychosocial issues and engage them on a path to take charge of their health—improving both their health status and reducing their overall cost of care. By analyzing a health plan’s population and segmenting to apply the appropriate member programs to meet the specific needs of each group, our health plan customers optimize their resource allocation and improve the health of their members. Our population health programs are complementary to medical management functions, establishing linkages and addressing the person across their needs.
Integrated Case Management
Integrated Case Management is designed to shepherd your most complex and costly members –those with several chronic medical conditions complicated by serious behavioral health conditions – through periods of acute or episodic care. Unique to this program is the ability to integrate traditional medical and behavioral health case management strategies and interventions together in a clinically integrated environment. The result is better outcomes, lower costs, and more capacity for your medical management team to work with your other members who are better served by traditional CM and DM programs.
Synergy Targeted Population Management®
Synergy is designed to target a health plan’s most costly, and challenging to manage, population segments—multi-chronic members with psychosocial issues that interfere with their ability to adhere to treatment plans, access appropriate care and make lifestyle changes. This segment is often missed by traditional disease management programs that do not adequately address psychosocial risks. With accurate risk identification, highly effective engagement and true empowerment to address medical and psychosocial issues, Health Integrated has achieved dramatic results for these member populations.
